D-12-5 was a member of Mobile Task Force D-12 "Mud Slingers" who was killed during his team's expedition into SCP-1730.

D-12-5 was dispatched alongside the rest of his team in front of SCP-1730's main entrance, and was the fourth to respond in the mic check conducted by D-12 Captain. Once inside the facility, Five confirmed that a service elevator Cap found had power and seemed functional.
After descending the elevator to Level B3, Five was ordered to stand watch by the shaft doors with D-12-2 as the rest of the team surveyed the floor. At some point shortly after, Five and Two were separated from their team and Provisional Site-23 Command's communications via a spatial anomaly. Displaced into a large utility room with boilers and a caved in wall, D-12-5 became suspended upside down at the mercy of giant leeches.
Five was horrified as he watched a leech slither down into Two's throat and kill his squadmate, exclaiming "Jesus Christ" as the leech made the corpse respond to the Site Command operator through the newly reconnected audio channel. As the operator explained to "Two" that the rest of the team had been compromised, another leech overwhelmed Five, burrowing into his chest and assuming control of his body. After the operator assured them that they were reestablishing their video feed, "Five" attempted to convince them to just send another team to extract them from SCP-1730 and/or get leeched. Video connection was reestablished, and the status of D-12-2's exsanguinated, hanging corpse was seen through D-12-5's body camera, along with a leech slithering on the wall behind them. Five's corpse opened its eyes before both audio-visual feeds cut out permanently.
